The second comma has lower priority than the other two: replace the other two with em-dash and then you come to today's reading.  Similar to XMLSchema's two-class wildcards :-) 

McCain was born US citizen to US citizen parents.  The exception with the commas was a grandfathering clause for any naturalized US citizen at the time. Apparently only John Adams's wife was such...
